Official Championship Roster & Liability Waiver Form (PDF Format)
This is the official ASA Championship Roster form that must be signed by all players and parents. Note: This form must be printed or copied and submitted as a two-sided document to Jim Donner Seattle/Tacoma Metro J.O. Commissioner.
Official Championship Tournament Entry Form (MS Word format)
This form must be filled out and signed by the ASA J.O Commissioner (Jim Donner) for all National qualifiers, Hall of Fame Qualifiers, National Qualifiers, Regionals, and National Championship Tournaments. This form will be presented to the tournament director before any championship play.

This form is required for ASA teams traveling out of state for tournament play.
Sample Medical Release (HTML format)
This is a sample Medical Release Form for your team's use. It is suggested that you have your attorney review it before using same. Although the ASA National Roster carries a similar form, it is recommended that all ASA traveling teams carry two sets for any medical emergency.
Player Agreement (HTML format)
An agreement between an individual player and a team. It is recommended that all team members and their parents sign this agreement upon joining a team.
